| 患者女,21岁,口周红斑、水疱15天,泛发全身伴瘙痒3天。既往有免疫性血小板减少病史。抗核抗体谱检查ANA 1：80,抗U1-nRNP(+),抗Sm抗体(+),抗Rib.P-Pr(+)。皮损组织病理及直接免疫荧光检查示：表皮下大疱形成,疱内见大量中性粒细胞,真皮浅层血管周围少许中性粒细胞、淋巴细胞浸润。免疫病理：直接免疫荧光(DIF)：基底膜线状IgG(+++)、IgA(+)、IgM(+)、C3(+/-)。间接免疫荧光(IIF)：BP-180阴性、Dsg-1阴性、Dsg-3阴性。诊断：大疱性系统性红斑狼疮。 |

| Abstract: |
| A 21-year-old woman with erythema and blisters around the mouth for 15 days, general pruritus for 3 days. Previous history of immune thrombocytopenia. Laboratory evalution revealed positive antinuclear antibody (titer 1:80), positive anti-U1-nRNP, positive anti-Sm antibody, and positive anti-Rib.P-Pr. Histopathology examination showed that a large number of neutrophils were found in the subepidermal bullae, a few neutrophils and lymphocytes infiltrate around the superficial dermal vessels. DIF revealed liner depositions of IgG(+++), IgA(+), IgM(+) , C 3 (+/ -) in the basement zone membrane. IIF revealed BP-180, Dsg-1 and Dsg-3 were negative. Diagnosis: bullous systemic lupus erythematosus. |
