目的 分析APACHES-Ⅱ、PCT、ALB等相关因素对慢性阻塞性肺疾病机械通气患者脱机、预后的影响。方法 根据2018年GOLD指南,将在我院RICU、ICU中进行有创机械通气的COPD患者在进行有创机械通气前行APACHE-II评分、PCT、CRP 等测定,同时记录患者一般情况,包括性别、年龄等相关资料,对成功脱机组、呼吸机依赖组、死亡组的患者相关因素水平进行比较、了解其有无统计学差异。结果 共纳入152例患者, 死亡组(28例)、脱机困难组(49例)及成功脱机组(75例)APACHE-II评分无显著性差异,与成功脱机组相比,死亡组与脱机困难组ALB显著降低(P<0.05或P<0.01),死亡组与脱机困难组PCT显著升高(P<0.05或P<0.01)。与死亡组相比,成功脱机组CRP显著降低(P<0.05);脱机困难组CRP有降低的趋势,但无显著性差异(P>0.05)。与成功脱机组相比,死亡组CRP显著升高(P<0.05);脱机困难组CRP有升高的趋势,但无显著性差异(P>0.05)。结论 APACHE-II评分在死亡组、脱机困难组及成功脱机组无显著性差异,PCT、ALB、CRP有一定的预测作用。 |

Objective to analyze the effects of APACHE-II, PCT, ALB and other related factors on weaning and prognosis in patients with chronic obstructive pulmonary disease (COPD). Methods according to the 2018 GOLD guidelines, COPD patients undergoing invasive mechanical ventilation in RICU and ICU in our hospital were measured APACHE-II score, PCT and CRP before mechanical ventilation, and the general situation of the patients, including sex, age and other related data, were recorded. The levels of related factors in the patients with successful removal of the unit, ventilator dependent group and death group were compared to see if there were statistical differences among 152 patients. There was no significant difference in APACHE-II score between the death group (28 cases), the weaning difficulty group (49 cases) and the successful removal group (75 cases). Compared with the successful removal group, the ALB in the death group and the difficult weaning group was significantly lower (P < 0.05 or P < 0.01). Compared with the successful removal group, PCT in the death group and the difficult weaning group was significantly higher than that in the control group (P < 0.05 or P < 0.01). Compared with the death group, the CRP of the successful removal group was significantly lower than that of the dead group (P < 0.05). The CRP of the difficult weaning group had a decreasing trend, but there was no significant difference between the two groups (P > 0.05). CRP in the death group was significantly higher than that in the control group (P < 0.05). There was no significant difference in CRP score between the two groups (P > 0.05). Conclusion there is no significant difference in APACHE-II score between the death group, the weaned difficulty group and the successful removal group. PCT, ALB and CRP can predict to a certain extent. |
