超声引导下液压扩张配合手法松解治疗冻结肩的疗效观察

摘要:
目的:探讨超声引导下液压扩张松解配合手法松解治疗冻结肩的临床疗效。 方法:将80例冻结肩患者随机分实验组(40例)和对照组(40例),实验组采用超声引导下液压扩张注射方法配合松解手法治疗,对照组予以超声引导下常规注射配合关节手法松解治疗,超声引导下液压扩张和常规注射治疗每周一次,手法松解治疗每天一次,治疗疗程为两周。治疗前后分别对患者数字疼痛评分(NRS)、Constant-Murley肩关节功能评分进行评估疗效。 结果:治疗后两组患者NRS疼痛评分差异具有统计学意义(实验组2.02±0.33,对照组3.84±0.61,P<0.05);两组患者治疗后Constant-Murley肩关节功能评分差异具有统计学意义(实验组:日常生活活动能力7.89±1.77、外展7.98±1.54、前屈7.79±1.15、外旋7.85±1.65、内旋8.04±1.57、手的位置8.72±1.81,对照组:日常生活活动能力7.01±1.46、外展6.94±1.23、前屈7.05±1.22、外旋7.31±1.04、内旋7.42±1.88、手的位置6.84±1.34,均P<0.05)。 结论:超声引导下液压扩张配合手法松解治疗技术是冻结肩治疗的有效方法, 具有创伤小、精准定位、并发症少、疗效可靠等优点,值得临床推广应用。

Effect of ultrasound guided hydrodilatation combined with manipulation release on frozen shoulder
Abstract:
Objective: To investigate the effect of ultrasound guided hydrodilatation combined with manipulation release in the treatment of frozen shoulder. Methods: 80 cases of patients with frozen shoulder were randomly divided into experimental group (40 cases) and control group (40 cases). The experimental group was treated with ultrasound guided hydrodilatation combined with manipulation release. The control group was treated with ultrasound guided injection combined with manipulation release. The treatment of ultrasound guided hydrodilatation was treated once a week, and manipulation release once per-day for two weeks. The Numeric rating scale and Constant-Murley shoulder joint function score were evaluated respectively before and after treatment. Results: The difference of NRS in the two groups was statistically significant (Experimental group 2.02±0.33, Control group 3.84±0.61, P<0.05). There were significant differences between the two groups of Constant-Murley shoulder function scores after treatment (Experimental group: Activity level 7.89±1.77, Abduction 7.98±1.54, Flexion 7.79±1.15, External rotation 7.85±1.65, Internal rotation 8.04±1.57, Positioning 8.72±1.81; Control group: Activity level 7.01±1.46, Abduction 6.94±1.23, Flexion 7.05±1.22, External rotation 7.31±1.04, Internal rotation 7.42±1.88, Positioning 6.84±1.34, P<0.05). Conclusion: Ultrasound guided hydrodilatation combined with manipulation release is an effective method for the treatment of frozen shoulder.
