【摘要】目的:观察腰椎动态关节松动术(Mulligan)结合PNF(Proprioceptive Neuromuscular Facilitation)技术对腰椎小关节紊乱的疗效观察。方法:将52名腰椎小关节紊乱的患者随机分为试验组和对照组各26例,两组都分别采用徒手腰椎动态关节松动技术(Mulligan),试验组同时增加PNF技术来增加躯干核心肌群与骨盆周围肌群稳定。两组均隔日治疗1次,共治疗 3次。每次治疗前采用Oswestry腰痛残疾问卷ODI(The Oswestry Disability Index)[[[]Fairbank J C, Pynsent P B. The Oswestry Disability Index.[J]. Spine, 2000, 25(22):2940.]]及治疗前、后采用视觉模拟评分法VAS(Visual Analogue Scale)评定治疗效果。结果:每位患者经过1周三次的治疗后,两组 VAS评分及ODI值均较治疗前明显下降(P<0.05),且试验组干预前后的差值更大((P<0.05),患者的疼痛症状明显缓解,其日常活动能力提高。结论:采用腰椎动态关节松动(Mulligan)结合PNF技术对腰椎小关节紊乱的疗效观察效果显著。明显优于徒手腰椎动态关节松动技术(Mulligan)。 |

Abstract
Objective: This study was conducted to examine the influence of combined exercise using proprioceptive neuromuscular facilitation (PNF) and Mulligan Mobilization Technique.
Methods: Fifty-two participants with Lumbar facet joint syndrome were randomly divided into 2 groups. The study group(aged 47.92 ± 14.96, 14men, 12women) received a conventional physical therapy program consisted of stretching and strengthening exercises plus SNAG (based on the Mulligan concept) on the affected lumbar levels, and the control group (aged 48.92 ± 13.59, 12 men, 14 women) received the same conventional program 3 times per week for 1 week. PNF technique was added to the control group to increase the stability of trunk core muscle group and peripelvic muscle group. Outcome measures were pain and function measured by visual analog scale, and the Oswestry Disability Index. Measurements were recorded before and after the end of the treatment period.
Result: After three times a week, the VAS score and ODI value of the two groups were significantly lower than those before treatment (P < 0.05). Besides, adding PNF to the conventional program resulted in higher improvement in terms of pain and function (P?<0.05) respectively.
Conclusion: This preliminary study indicated improvement in both groups. Mulligan mobilization technique combined with PNF technique is obviously superior to the Mulligan mobilization technique. |
