目的:探讨血清miR-145-5p水平与住院AECOPD患者病情严重程度的关系。
方法:将我院2018年01月至2019年03月收治的168例慢性阻塞性肺疾病急性加重期(AECOPD)患者,根据患者肺功能FEV1%及血气分析结果分为两组,非重度AECOPD组(n=112)和重度AECOPD组(n=56)。记录患者入院时的基本信息、住院期间死亡率、血气分析结果、血常规结果、血清PCT和CRP水平。血清TNF-α和IL-8水平使用ELISA进行检查。血清miR-145-5p水平使用qPCT进行检测。
结果:与非重度AECOPD组患者相比较,重度AECOPD组患者死亡率、血气指标中PaCO2和剩余碱(BE)值、白细胞数(WBC)、中性粒细胞数(NS)、血清PCT、CRP、TNF-α和IL-8水平明显升高,体重指数(BMI)、血气指标中PH、PaO2和血清miR-145-5p水平明显下降,差异均有显著统计学差异(P均<0.05)。进一步发现PaCO2与miR-145-5p水平呈显著负相关性(r =-0.512)。
结论:血清miR-145-5p水平与住院AECOPD患者的病情严重程度密切相关,且与CO2潴留有较好的相关性,具有成为评估AECOPD严重程度、治疗效果及预后的分子标志物的潜在价值。但仍需进一步的大规模研究进一步证实其实际意义。 |

The correlation between the severity of AECOPD and the serum level of miR-145-5p |

Abstract: |
Objective To study the correlation between the severity of disease and the serum level of miR-145-5p in the patients with hospitalized AECOPD.
Methods In this clinical study, 168 patients with hospitalized AECOPD were included and divided into 2 groups, Non-Severe AECOPD group (n=112) and Severe AECOPD group (n=46), from January 2018 to March 2019. The demographic information, mortality, air blood gas (ABG) indexes, blood routine indexes, the serum levels of PCT and CRP were recorded. Then, the serum levels of TNF-α and IL-8 were measured by ELISA. The serum level of miR-145-5p was analyzed by qPCR.
Results Compared with Non-Severe AECOPD group, the morality, white blood cells (WBC), neutrophils (NS), PaCO2, base excess (BE) value, the serum levels of TNF-α and IL-8 were significantly increased, and, BMI, PH, PaO2, and the serum level of miR-145-5p were noticeably reduced in Severe AECOPD group (P <0.05 in all). Good correlation between PaCO2 and the serum level of miR-145-5p was found (r=-0.512).
Conclusions The correlation of the severity of AECOPD and the serum level of miR-145-5p was good. The serum level of miR-145-5p probably can be used as a potential biomarker to evaluate the severity of the disease, to assess the effective of the treatment, to predict the prognosis of the patients with AECOPD. However, the large scale study is needed to further confirm its practical value. |
