耐甲氧西林金黄色葡萄球菌的临床分布及agr基因分型调查

摘要:
目的了解我院金黄色葡萄球菌的临床分布、耐药性变迁及agr基因分型。方法我院临床分离的耐甲氧西林金黄色葡萄球菌,采用微量肉汤法进行药敏试验和结果判断;采用多重PCR法进行agr基因分型测定。结果 2015年10月至2017年3月分离的183株耐甲氧西林金黄色葡萄球菌主要来源于24个科室,感染标本来源主要是分泌物、深部痰及非开放性脓液标本,药敏试验提示万古霉素、利奈唑胺、替加环素、复方新诺明、利福平、莫西沙星、左氧氟沙星、环丙沙星、红霉素对耐甲氧西林金黄色葡萄球菌敏感率分别为100%、100%、100%、83.06%、62.29%、57.38%、55.19%、53.0%和12.57%;agr基因分型以Ⅰ型为主,其次为Ⅱ型、Ⅲ型。结论我院耐甲氧西林金黄色葡萄球菌于全院均匀分布,对红霉素耐药率较高,未发现对万古霉素、利奈唑胺、替加环素耐药株。

Clinical Distribution and Agr Genotyping Survey of Methicillin Resistant Staphylococcus Aureus

Abstract:
Objective To investigate the clinical distribution,changes of drug resistance and agr genotyping of Staphylococcus aureus in our hospital. Methods Methicillin resistant Staphylococcus aureus clinically isolated from our hospital in 2016 was performed drug sensitivity test by the method of micro meat soup and the result was judged; Agr genotyping testing was performed by multiplex PCR method.Results 183 strains of methicillin resistant Staphylococcus aureus isolated from October 2015 to March 2017 were mainly derived from 24 departments,and the main sources of infection specimens were secretions,deep sputum and non open pus specimens.Drug sensitivity tests showed that the sensitive rate of vancomycin,linezolid,tegafur,sulfamethoxazole,rifampicin,moxifloxacin,levofloxacin,ciprofloxacin and erythromycin for methicillin resistant Staphylococcus aureus were 100%、100%、100%、83. 06%、62. 29%、57. 38%、55. 19%、53%、12. 57%,respectively; The genotype of agr was mainly type Ⅰ,followed by type Ⅱ and type Ⅲ.Conclusion Methicillin resistant Staphylococcus aureus in our hospital was evenly distributed in the whole hospital,and the drug resistance rate of erythromycin was higher. No vancomycin,linezolid,and cyclosporine resistant strains were found.
