宫颈液基超薄细胞技术联合LEEP术对宫颈上皮内瘤变的诊疗价值

摘要:
目的探讨宫颈液基超薄细胞技术(TCT)与LEEP联合在宫颈上皮内瘤变(CIN)中的诊疗价值。方法选取本院妇科门诊2011年1月至2012年1月接诊的疑为CIN患者275例,TCT检查后与术后病理结果进行对比分析,探讨TCT对CIN诊断价值,并对确诊患者采用LEEP治疗,探讨其治疗效果。结果 TCT检查与术后病理结果比较差异无统计学意义(P>0.05),TCT对CIN的诊断敏感度为96.7%,特异度为57.3%,诊断准确性为79%。2例浸润癌患者行广泛子宫切除术+盆腔淋巴结清扫术。随访2年,CIN治愈率为91.63%,复发率为8.3%,23例复发患者再次LEEP治疗后治愈。结论在宫颈上皮内瘤变诊断及筛查中具有较高的诊断准确性,同时LEEP技术在宫颈上皮内瘤变患者治疗中疗效确切,安全可靠。

Clinical Value of Liquid Based Cytology Combined with LEEP in the Diagnosis and Treatment of Cervical Intraepithelial neoplasia

Abstract:
Objective To investigate the value of cervical Thin Prep cytology technique( TCT) combined with LEEP in the diagnosis and treatment of cervical intraepithelial neoplasia( CIN).Methods 275 cases of suspected CIN patients admitted to the outpatient department of gynecology in our hospital from January 2011 to January 2012 were selected.After TCT examination,the results were compared and analyzed with postoperative pathological results to explore the value of TCT in the diagnosis of CIN. And patients with confirmed CIN were treated with LEEP,and the treatment effect was discussed. Results There was a significant difference between TCT examination and postoperative pathological findings( P>0. 05).The sensitivity of TCT to CIN was 96. 7%,the specificity was 57. 3%,and the diagnostic accuracy was 79%.2 patients with invasive carcinoma underwent extensive hysterectomy and pelvic lymph node dissection.After 2 years of follow-up,the cure rate of CIN was 91. 63%,the recurrence rate was 8. 3%,and 23 relapsed patients were cured after LEEP treatment again.Conclusion The combination of TCT and LEEP has high diagnostic accuracy in the diagnosis and screening of CIN.Meanwhile,LEEP technology is effective and safe in the treatment of CIN
