目的采用复吸法逐步增加吸入气二氧化碳浓度,观察不同临床剂量右美托咪定对高二氧化碳通气驱动反应的影响。方法将32例患者随机分为4组,生理盐水组、右美托咪定0.2μg/kg、0.5μg/kg和1.0μg/kg组。严格按照实验流程,分别记录呼气末二氧化碳分压(PETCO2)等于50mm Hg、55mm Hg和60mm Hg时每位受试者的呼吸频率(RR,次/min)、潮气量(VT,m L)以及分钟通气量(MV,L/min)。结果随着PETCO2升高,四组患者的RR、VT和MV均显著增加,差异有统计学意义(P<0.001)。当PETCO2逐渐增加至55mm Hg和60mm Hg时,与生理盐水组相比,右美托咪定0.2μg/kg组不会减少患者RR、VT以及MV;但右美托咪定0.5μg/kg和1.0μg/kg组在不减慢患者RR情况下,能显著减少患者VT和MV。结论右美托咪定0.5μg/kg和1.0μg/kg能显著减弱高二氧化碳通气反应,推测可能主要通过减少机体潮气量而实现。 更多 |

Objective The inspired concentration of CO2 was gradually increased using rebreathing,we aimed to explore the effect of dexmedetomidine on the ventilatory response to hypercapnia in humans. Methods Thirty-two patients was randomly divided into 4 groups,control group and dexmedetomidine 0. 2、0. 5 and 1. 0μg/kg groups. According to protocol,we respectively measured the respiratory rate( RR),vidal volume( VT) and minute ventilation( MV) of patients when PETCO2= 50,55 and 60 mmHg.Results With the increase of PETCO2,the RR,VT and MV of patients were significantly raised.When PETCO2= 55 or 60 mmHg,compared with control group,dexmedetomidine 0. 2 μg/kg didn’t reduced the RR,VT and MV of patients; but dexmedetomidine 0. 5 and 1. 0μg/kg didn’t slow down the patient RR and significantly reduced the patient VT and MT. Conclusion Dexmedetomidine 0. 5 and 1. 0μg/kg can significantly attenuate the ventilatory response to hypercapnia by reducing vidal volume in humans. 更多 |
