| 目的评估精索静脉结扎术(MSV)术前和术后睾丸动脉(TA)、睾丸包膜动脉(CA)和睾丸内动脉(ITA)的收缩期峰值速度(PSV)和阻力指数(RI)差异。方法 66例(17~35岁)精索静脉曲张患者分别在术前、术后3个月以及术后6个月行阴囊彩色多普勒超声(CDUS)检查。分析多普勒对手术疗效的预估价值。结果术后第一次睾丸多普勒显示TA-PSV显著增加(P<0.001),TA-RI降低(P<0.001),CA-RI降低(P<0.001)。术后第二次睾丸多普勒也显示TA-PSV显著增加,TA-PSV降低,同时ITA-PSV、CA-PSV、ITA-RI、CA-RI均与术前及术后第一次测量有显著差异。结论 PSV和RI两指标可用来检测精索静脉结扎术后睾丸微循环血流动力学变化,TA-RI,TA-PSV和CA-PSV可能与疼痛和精子密度的改善相关。 |

| Objective To evaluate the difference of peak systolic velocity( PSV) and resistance index( RI) of testicular artery( TA),testicular capsule artery( CA) and intra-testicular artery( ITA) before and after spermatic vein ligation( MSV).Methods Color Doppler ultrasonography( CDUS) examination was performed in 66 patients( 17-35 years old) with varicocele before operation,3 months after operation and 6 months after the operation.The predictive value of testicular Doppler for surgical outcomes was analysed.Results The first testicular Doppler examination after operation showed that TA-PSV increased significantly( P < 0. 001),TA-RI decreased( P<0. 001) and CA-RI decreased( P < 0. 001). Second times after operation,testicular Doppler examination also showed that TA-PSV increased significantly and TA-PSV decreased.Meanwhile,ITA-PSV,CA-PSV,ITA-RI and CA-RI were significantly different from results of preoperative and postoperative first measurement.Conclusion Both PSV and RI can be used to detect the hemodynamic changes of testicular microcirculation after varicocele ligation.TA-RI,TA-PSV and CA-PSV may be related to the improvement of pain and sperm density |
