| 目的通过进行为期1年的PDCA循环科学质量管理后,验证其在抗菌药物临床应用监测(MCUA)管理中的效果,为临床合理用药提供参考。方法从2015年1月至12月,应用经典的戴明环(PDCA)管理方法,使MCUA工作制度化、程序化,并将管理前后临床抗菌药物的使用情况进行比较,考察PDCA循环管理的合理性。结果通过PDCA循环,按主要科室统计基本抗菌药物使用处方有明显提高,平均由24.88%上升至44.69%,同时基本抗菌药物比例由干预前的24.19%上升至45.01%;通过干预使基本抗菌药物的使用比例进入前五位,且结构更加合理。门诊抗菌药物处方合格率由82.42上升至99.17%,且临床科室对处方点评的反馈明显改善。结论 PDCA管理可用于提高MCUA管理水平,对促进临床安全、有效、经济地应用抗菌药起到积极作用。 |

| Objective After 1 year of scientific quality management of PDCA cycle,the effect of its management in the clinical usage of antimicrobial agents( MCUA) was verified to provide a reference for clinical rational drug use. Method From January to December in 2015,MCUA work was institutionalized and programmed by the application of classic PDCA circle management.The clinical usage of antibacterial agents were examined before and after the management of PDCA cycle to evaluate the rationality of it.Results Through the PDCA cycle,the quantity of prescriptions of basic antimicrobial agents in the main departments significantly increased from 24. 88% to 44. 69%,while the proportion of basic antibacterial drugs increased from 24. 19% to 45. 01%. The proportion of basic antimicrobial agents was in the top five by the intervening of PDCA,and the structure is more reasonable.The prescription rate of outpatient antimicrobial drugs increased from 82. 42% to 99. 17%,and the clinical department’s feedback on prescription comments improved significantly. Conclusion PDCA management can be used to improve MCUA management level and play an active role in promoting clinical safety,effective and economic application of antimicrobial drugs |
