| 目的对手术治疗在老年喉癌方面的临床疗效及其相关影响因素进行探究。方法取2007年4月至2012年3月在我院耳鼻喉科接受治疗的180例喉癌患者进行回顾分析,统计分析每位患者术后生存率情况并对其影响因素进行单因素、多因素分析。结果 180例喉癌患者术后1、3、5年生存率分别为96.67%(174/180)、70.56%(127/180)、51.67%(93/180)。单因素分析表明,肿瘤大小、病理分级、年龄、肿瘤原发位置、是否淋巴结转移以及治疗方案为明显的影响预后因素。多因素分析发现,影响喉癌预后的独立因素有肿瘤原发位置、治疗方案、TNM分期以及是否淋巴结转移。结论老年喉癌患者的早期诊断与及时手术是提高患者生存率的关键,患者淋巴结转移情况、肿瘤原发位置、手术方案以及TNM分期为影响喉癌患者预后最主要的独立因素,应予重视。 |

| Clinical Observation and Influencing Factors of Surgical Treatment for Elderly Patients with Laryngeal Cancer |

| Objective To investigate the factors of surgical treatment and its related aspects in aged patients with laryngeal carcinoma.Methods Over the past five years( 2012.4-2017.3),180 cases of laryngeal cancer patients treated in our ENT department were retrospectively analysized,postoperative survival rate,single and multivariate factors were analysized. Results 1,3,5 years survival rate was 96.67%( 174/180) 、70. 56%( 127/180) 、51. 67%( 93/180) respectively. The single factor analysize showed that the tumor size,pathological grade of laryngeal cancer,the age of patients,the cancer location,lymph node metastasis and treatment were prognostic factors. Multivariate analysis showed that the independent factors affecting the prognosis were the cancer location,treatment,TNM staging and lymph node metastasis. Conclusion Early diagnosis and timely surgical treatment for elderly patients with laryngeal cancer is the key to improve the survival rate of the patients. Lymph node metastasis,cancer location,operation scheme and TNM staging were the most important independent factor affecting the prognosis of laryngeal cancer patients |
