目的分析脑动脉粥样硬化患者血脂、修饰化脂蛋白及脂蛋白相关磷脂酶A2水平,研究修饰化脂蛋白及脂蛋白相关磷脂酶A2与脑动脉粥样硬化发生的关系。方法以2016年5月至2016年9月因缺血性脑卒中就诊于华西医院神经内科确诊为脑动脉粥样硬化患者共183例为病例组,以同期华西医院体检中心健康体检者180例为对照组,采用酶联免疫吸附法测定修饰化脂蛋白水平,比较病例组与对照组修饰化脂蛋白水平差异。结果脑动脉粥样硬化组血糖水平为6.14(5.21,7.67)(mmol/L)高于健康对照组的4.84(4.68,5.21)(P<0.05),甘油三酯在两组间无差异(P>0.05),病例组胆固醇、低密度脂蛋白-胆固醇、高密度脂蛋白-胆固醇水平均低于对照组,结果为3.57(3.07,4.17)vs 5.24(4.86,5.75)(mmol/L);1.83(1.46,2.30)vs 2.97(2.58,3.23)(mmol/L);1.11(0.84,1.36)vs 1.43(1.17,1.76)(mmol/L)(P<0.05)。病例组氧化高密度脂蛋白、氧化低密度脂蛋白、糖化高密度脂蛋白、糖化低密度脂蛋白、脂蛋白相关磷脂酶A2水平均对照组,结果为75.48(68.56,78.55)vs 40.60(32.76,47.92)(ng/mL);15.81(11.29,19.07)vs 12.22(11.46,13.38)(μg/mL);4.79(3.77,6.48)vs 2.55(1.86,4.33)(ng/mL);31.20(24.39,40.56)vs 24.69(19.16,33.19)(ng/mL);7.73(5.81,10.35)vs 4.20(3.62,5.42)(ng/mL),差异有统计学意义(P<0.05)。结论脂质不是脑动脉粥样硬化形成的主要危险因素,高血糖和修饰化脂蛋白及脂蛋白相关磷脂酶A2水平升高是脑动脉粥样硬化发生发展的重要因素。 |

Objective To analyze the levels of blood lipids,modified lipoproteins and lipoprotein-associated phospholipase A2 in patients with cerebral atherosclerosis and investigate the relationship between modified lipoproteins and lipoprotein-associated phospholipase A2 and cerebral atherosclerosis.Methods 183 cases diagnosed as cerebral atherosclerosis admitted to West China Hospital for ischemic stroke from May 2016 to September 2016 were selected as the case group and 180 cases of healthy subjects in Physical Examination Center of West China Hospital during the same period were selected as the control group.the level of modified lipoprotein was measured by enzyme-linked immunosorbent assay( ELISA),and the difference of modified lipoprotein between the case group and the control group was compared.Results Blood glucose level in cerebral atherosclerosis group was 6. 14( 5. 21,7. 67)( mmol/L),higher than that in healthy control group 4. 84( 4. 68,5. 21) with statistical significance( P <0. 05).There was no difference in triglyceride between the two groups( P > 0. 05). The levels of cholesterol,low density lipoprotein-cholesterol and high-density lipoprotein-cholesterol were lower in the case group than those in the control group with statistical significance( P<0. 05),and the results were 3. 57( 3. 07,4. 17) vs 5. 24( 4. 86,5. 75)( mmol/L); 1. 83( 1. 46,2. 30) vs 2. 97( 2. 58,3. 23)( mmol/L); 1. 11( 0. 84,1. 36) vs 1. 43( 1. 17,1. 76)( mmol/L)( P<0. 05).The levels of oxidized high density lipoprotein,oxidized low density lipoprotein,glycated high density lipoprotein,glycosylated low density lipoprotein and lipoprotein associated phospholipase A2 in the case group were significantly higher than those in the control group( P < 0. 05),and the results were 75. 48( 68. 56,78. 55) vs 40. 60( 32. 76,47. 92)( ng/mL); 15. 81( 11. 29,19. 07) vs 12. 22( 11. 46,13. 38)( μg/mL); 4. 79( 3. 77,6. 48) vs 2. 55( 1. 86,4. 33)( ng/mL); 31. 20( 24. 39,40. 56) vs 24. 69( 19. 16,33. 19)( ng/mL); 7. 73( 5. 81,10. 35) vs 4. 20( 3. 62,5. 42)( ng/mL) respectively. Conclusion Lipid is not the main risk factor of cerebral atherosclerosis,but the high level of phospholipase A2,hyperglycemia,modified lipoprotein are important factors in the development of cerebral atherosclerosis |
