目的探讨颈动脉斑块积分、血浆同型半胱氨酸(Hcy)水平与冠状动脉病变严重程度的关系。方法选择2012年10月至2016年3月因疑似冠心病(CHD)症状被收入我院患者774例,根据冠脉造影结果被分为非冠心病(NCHD)组393例与冠心病(CHD)组381例,CHD组根据冠脉Gensini评分又被分为低危组181例(<20分)、中危组116例(20~40分)、高危组84例(>40分),分析患者颈动脉斑块积分、血浆Hcy水平与冠脉Gensini评分之间的关系。结果CHD组颈动脉斑块积分(9.68±3.93)及血浆Hcy水平(16.35±5.49)μmol/L均高于NCHD组[(4.19±1.43)、(11.36±4.91)μmol/L],颈动脉斑块积分与Hcy水平在CHD亚组间两两比较,差异均有统计学意义(P<0.05),且斑块积分、Hcy水平与冠脉Gensini评分呈正相关性(r=0.761、0.637,P<0.01)。结论 CHD患者颈动脉斑块积分与Hcy水平检测有助于判断冠状动脉病变的严重程度。 |

Objective To investigate thecorrelation between carotid plaque score,plasma homocysteine( Hcy) levels and the severity of coronary artery disease.Methods Between October 2012 and March 2016,774 patients with suspected coronary artery disease( CHD) symptoms were selected from our hospital,and they were divided into non coronary artery disease( NCHD)group of 393 cases and coronary artery disease( CHD) group of 381 cases according to the coronary angiography results. Among them,CHD group was divided into low-risk group of 181 cases( <20 points),middle risk group of 116 cases( 20 to 40 points),high-risk group of 84 cases( >40 points) according to coronary Gensini score. And the relationship between carotid plaque score,plasma Hcy level and coronary Gensini score was analyzed. Results Carotid plaque score( 9. 68 ± 3. 93) and plasma Hcy level( 16.35±5.49) μmol/L in group CHD were higher than those in group NCHD[( 4.19±1.43) 、( 11.36±4.91) μmol/L]( P<0.05);There was significant difference between the scores of carotid plaques and the level of Hcy in the comparison of subgroups of CHD( P<0.05),and the plaque score,Hcy level and coronary Gensini score were positively correlated( r = 0.761、0.637,P<0.01).Conclusion Detection of carotid plaque score and Hcy level in patients with CHD can help to determine the severity of coronary artery disease |
