| 目的探讨鼻内镜治疗慢性鼻窦炎伴鼻息肉的临床效果。方法选取2014年2月至2016年4月在我院治疗的慢性鼻窦炎伴鼻息肉患者92例,均采用鼻内镜手术治疗,观察随访治疗效果及复发等情况,采用T&T标准试嗅法评估手术前后患者嗅觉功能,SF-36中文版量表评估患者手术前后生活质量。结果术后3个月随访,治愈率为68.48%;术后12个月随访,治愈率为86.96%;术后12个月患者嗅觉功能明显好于术后3个月和术前,差异有统计学意义(P<0.05);术后3个月患者嗅觉功能情况与术前比较差异无统计学意义(P>0.05);术后3个月和12个月患者生理角色、机体疼痛、活力、生活功能、心理角色、心理健康等维度得分和SF-36总分均较术前明显提高,且术后12个月明显好于术后3个月,差异有统计学意义(P<0.05);治疗期间患并发症发生率为9.78%;截止随访结束,共有4例患者鼻窦炎复发,复发率为4.35%。结论鼻内镜治疗鼻窦炎伴鼻息肉是安全可靠的,能改善患者嗅觉和生活质量,且复发率低,值得临床推广应用。 |

| Objective To explore the clinical effect of endoscopic sinus surgery for chronic sinusitis with nasal polyps.Methods 92 cases with chronic sinusitis and nasal polyps receiving surgery were selected from February 2014 to April 2016 in our hospital and the curative effect and long-term recurrence rate were observed. T & T criteria were used to evaluate the olfactory function of patients before and after surgery. The Chinese version of SF-36 was used to evaluate the quality of life before and after operation.Results The cure rate was 68. 48% at 3 months after operation; The cure rate was 86. 96% at 12 months after operation; The olfactory function at 12 months after operation was better than that after 3 months and before the operation,statistically significant( P<0. 05); There was no significant difference between the olfactory function at 3 months after operation and that before the operation( P > 0. 05); The physiological scores,body pain,vitality,life function,psychological role,mental health and other scores of SF-36 at 3 months and 12 months postoperatively were significantly higher than those before operation,and the effect at 12 months after operation were significantly better than that at 3 months after operation,statistically significant( P< 0. 05); The incidence of complications during treatment was 9. 78%; At the end of follow-up,4 patients had recurrence of sinusitis and the recurrence rate was 4. 35%. Conclusion Endoscopic sinus surgery for chronic sinusitis with nasal polyps is safe and reliable.It can not only improve the patients’ olfactory and quality of life but also have low recurrence rate,which is worthy of clinical application. |
