目的评估稳定期慢阻肺患者间歇性使用无创机械通气治疗后患者血清超敏C反应蛋白(hs CRP)及白介素8(IL-8)表达的改变,进而评估无创通气对慢阻肺气
道炎症的影响。方法连续选取2015年7月—2016年10月中国医科大学附属第一医院呼吸内科和大连医科大学附属第一医院呼吸内科随诊的中重度稳定期COPD患者
20名,给予患者稳定期家庭式间歇无创通气治疗。治疗过程中的随访及管理由大连旅顺口区医院及沈阳翟家社区卫生服务中心协助完成。留取间歇无创通气治疗前
及无创通气治疗2个月后受试者空腹静脉血清用于检测hs CRP,IL-8,同时比较治疗前及治疗2个月后血气分析指标变化。结果本研究共纳入20例稳定期慢阻肺患者
,其中肺功能分级Ⅲ级11例,Ⅳ级9例;男13例,女7例。家庭式间歇无创机械通气治疗2个月后患者二氧化碳分压(Pa CO2)及患者CAT及圣乔治评分均较治疗前有所改
善,差异有统计学意义(P<0.05);治疗前后患者血清IL-8、hs CRP的水平无明显变,差异无统计学意义(P>0.05)。结论稳定期慢阻肺患者使用家庭式间歇性无创正
压通气治疗可改善患者通气状态,使动脉血氧分压增加,二氧化碳分压降低,症状改善,且无创通气未增加机体的炎症因子水平。 |

Objective To evaluate the changes of serum high sensitive C-reactive protein( hs CRP) and interleukin 8( IL-8) expression in patients with chronic obstructive pulmonary disease( COPD) during stable phase after use of intermittent non-invasive mechanical ventilation,and to further evaluate the effect of non-invasive ventilation on airway inflammation in COPD patients.Methods 20 patients with moderate and severe COPD during stable phase followed by department of respiratory medicine,the First Affiliated Hospital of China Medical University and department of respiratory medicine,the First Affiliated Hospital of Dalian Medical University during July 2015 and October 2016,were continuously selected and given family intermittent non-invasive ventilation.The follow-up and management in the course of treatment is supported by Lushunkou District Hospital of Dalian and Zhaijia community health service center of Shenyang. Fasting venous blood of the subjects were collected before intermittent non-invasive ventilation and after 2-months treatment of non-invasive ventilation,and were used to determine hs CRP and IL-8.Simultaneously,blood gas analysis indicators before and after 2-months treatment were compared.Results 20 cases of COPD patients during stable phase were included in this study,among which there were 11 cases of gradeⅢ pulmonary function and 9 cases of gradeⅣpulmonary function,13 females and 7 males.After 2-months treatment of family intermittent non-invasive mechanical ventilation,partial pressure of carbon dioxide( Pa CO2) and scores of CAT and St Giorgio Respiratory Questionnaire in patients were improved compared with those before treatment,the difference being statistically significant( P<0. 05).Serum IL-8 and hs CRP of patients before and after treatment showed no statistically significant difference( P> 0. 05). Conclusion The use of family intermittent non-invasive positive pressure ventilation in stable COPD patients can improve the vented condition,increase the arterial partial pressure of oxygen,reduce the partial pressure of carbon dioxide and improve symptoms. Furthermore,non-invasive ventilation does not increase the level of inflammatory factors. |
