代谢综合征与泌尿系结石的危险因素及结石成分的关系

摘要:
目的探讨代谢综合征(MS)与泌尿系结石的危险因素并分析其与结石成分的关系。方法选取2015年10月至2016年11月我院收治的150例泌尿系结石患者为研究对象,将患者分为合并MS组和单纯结石组,比较两组血液及结石指标差异,采用单因素和多因素Logistic回归分析MS与泌尿系结石的独立危险因素,并采用Spearman相关性分析MS与结石成分的关系。结果合并MS组BMI、收缩压、舒张压、空腹血糖、TG、尿酸表达水平较单纯结石组明显高(P<0.05);合并MS组草酸钙结石明显少于单纯结石组,磷酸钙结石、尿酸钙结石较单纯结石组明显多(P<0.05);经多因素Logistic回归分析结果显示高BMI、高空腹血糖、高尿酸及尿酸结石多是MS与泌尿系结石的独立危险因素(P<0.05);而Spearman相关性分析MS与尿酸钙结石呈明显正相关,其与草酸钙结石呈明显负相关(r=0.360、-0.315,P<0.05),但与磷酸钙结石、磷酸镁铵结石、胱氨酸结石无明显相关性。结论 MS与泌尿系结石患者体重、尿酸含量、尿结石成分中尿酸结石增多和草酸钙结石减少等危险因素紧密相关,临床应据此为患者实施针对性预防、治疗措施。

Abstract:
Objective To investigate the risk factors of metabolic syndrome( MS) and urinary calculus and to analyze the relationship between MS and stone composition.Methods From October 2015 to November 2016,150 patients with urinary calculus treated in our hospital were selected as the research subjects and randomly divided into two groups : combined MS group and simple stone group.The differences of blood and stone indexes between the two groups were compared,and the independent risk factors of urinary calculus were analyzed by univariate and multivariate Logistic regression analysis.Spearman correlation was used to analyze the relationship between MS and stone composition.Results The levels of BMI,systolic pressure,diastolic pressure,fasting blood glucose,TG,and uric acid expression in the combined MS group were significantly higher than those in the simple stone group( P<0. 05); Calcium oxalate calculi in MS group were significantly less than those in simple stone group,while calcium phosphate stone and calcium uric acid stone in MS group were significantly more than those in simple stone group( P< 0. 05); The results of multivariate Logistic regression analysis showed that high BMI,high fasting blood glucose,high uric acid and uric acid stone were the independent risk factors of MS and urinary calculus( P< 0. 05); Spearman correlation analysis showed that MS was positively correlated with calcium uric acid stone and negatively correlated with calcium oxalate stone( r = 0. 360、-0. 315,P < 0. 05),but there was no significant correlation with MS,calcium phosphate stone,magnesium ammonium phosphate stone and cystine stone.Conclusion Ms was closely related to the risk factors such as body weight,uric acid content,uric acid stone increase and calcium oxalate stone reduction in patients with urinary calculus. According to this,we should carry out targeted prevention and treatment measures for the patients in clinical work
