| 目的观察内镜套扎术联合改良三明治法注射治疗食管胃底静脉曲张出血的近远期疗效。方法选取我院99例胃底食管静脉曲张患者,通过随机数字表法分为套扎组,注射组和联合治疗组(每组各33例),套扎组单独使用内镜套扎术,注射组单独使用组织胶与聚桂醇的改良三明治法注射,联合治疗组联合使用内镜套扎术和组织胶与聚桂醇的改良三明治法注射,观察比较三组的治疗有效率,止血成功率,近期再出血和并发症,以及两年后随访情况。结果经过治疗后,联合治疗组的治疗有效率为93.93%,明显高于其他两组,差异具有统计学意义(P<0.05)。联合治疗组的止血成功率,近期再出血和并发症,也明显优于其他两组,差异有统计学意义(P<0.05)。在两年后的随访结果中,联合治疗组也明显优于其他两组,差异具有显著统计学意义(P<0.05)。结论内镜套扎术联合改良三明治法注射治疗食管胃底静脉曲张出血的近期和远期疗效都十分显著,值得临床推广。 |

| Objective To observe the short-term and long-term therapeutic effect of endoscopic ligation combined with modified Sanming injection in the treatment of esophagogastric variceal bleeding.Methods 99 patients with gastric fundus and esophageal varices in our hospital were selected and divided into the ligation group,the injection group and the combined treatment group by random number table method( 33 cases in each group).The ligation group was treated by endoscopic ligation alone,the injection group was treated with modified Sanming injection by tissue glue and polylcinol,and the combined treatment group was treated by endoscopic ligation and modified Sanming therapy with tissue glue and polylcinol. The efficiency of treatment,the success rate of hemostasis,the recent rebleeding and complications,and the follow-up conditions after 2 years were observed and compared among the three groups.Results After treatment,the effective rate of the treatment of combined treatment group was 93. 93%,significantly higher than the other two groups,and the difference was statistically significant( P< 0. 05). The hemostasis success rate,the recent rebleeding and complications of the combined treatment group were obviously superior to those of the other two groups,statistically significant( P<0. 05).In the follow-up results after 2 years,the combination treatment group was also significantly superior to the other two groups,with a statistically significant difference( P<0. 05).Conclusion The short-term and long-term effect of endoscopic ligation combined with modified Sanming injection in the treatment of esophagogastric variceal bleeding are very significant,which is worthy of clinical application |
