| 目的研究并比较载脂蛋白B/载脂蛋白A-I(ApoB/ApoA-I)、非高密度脂蛋白/高密度脂蛋白(NHDL-C/HDL-C)2种血清脂蛋白比值对2型糖尿病患者非糖尿病一级亲属发生代谢综合征(MS)的诊断预测价值。方法选择来自2型糖尿病家系的非糖尿病一级亲属327例,测定身高、体质量、血压、血脂、血糖等临床和生化指标,计算ApoB/ApoA-I、NHDL-C/HDL-C、BMI等指数。分析ApoB/ApoA-I、NHDL-C/HDL-C比值与MS各组分的相关性。ROC曲线确定两者诊断MS的临界值,并比较两者对2型糖尿病患者非糖尿病一级亲属发生MS的诊断预测效果。结果 ApoB/ApoA-I、NHDL-C/HDL-C比值随代谢综合征异常指标项数的增加而升高,且与代谢综合征各组分均明显相关。ApoB/ApoA-I、NHDL-C/HDL-C的ROC曲线下面积分别为0.743、0.822(P<0.01)。结论 ApoB/ApoA-I、NHDL-C/HDL-C比值均与MS及其组分密切相关,均可单独作为MS发生发展的一个危险因素。NHDL-C/HDL-C对2型糖尿病患者非糖尿病一级亲属发生MS的预测价值优于ApoB/ApoA-I。 |

| Objective To study and compare the predictive value of the two serum lipoprotein ratio: apolipoprotein B/apolipoprotein A-I( ApoB/ApoA-I) and non high density lipoprotein/high-density lipoprotein( NHDL-C/HDL-C),in the diagnosis of non diabetic first-degree relatives metabolic syndrome in type 2 diabetic patients. Methods We chose 327 non diabetic first-degree relatives from type 2 diabetes families,and measured the clinical and biochemical indexes such as height,body weight,blood pressure,blood lipid and blood sugar,and calculated indexes such as ApoB/ApoA-I,NHDL-C/HDL-C and BMI.The correlation between the ratio of ApoB/ApoA-I and NHDL-C/HDL-C and the components of MS was analyzed.The ROC curve was used to determine the critical value of the diagnosis of MS and to compare the predictive effect in the diagnosis of non diabetic first-degree relatives metabolic syndrome in type 2 diabetic patients.Results The ratio of ApoB/ApoA-Igna NHDL-C/HDL-C increased with the increase of abnormal items of metabolic syndrome,and was significantly related to each component of metabolic syndrome. The area under the ROC curve of ApoB/ApoA-Ig NHDL-C/HDL-C was 0. 743、0. 822( P < 0. 01). Conclusion The ratio of ApoB/ApoA-I and NHDL-C/HDL-C is closely related to MS and its components,which can be used as a risk factor for the development of MS alone.The predictive value of NHDL-C/HDL-C to MS in non diabetic first-degree relatives of type 2 diabetic patients is better than that of ApoB/ApoA-I |
