| 目的探讨糖皮质激素联合复方异丙托溴胺对慢性阻塞性肺疾病急性加重期(AECOPD)患者的治疗效果。方法选取2016年1月至2016年12月在我院接受治疗的AECOPD患者为研究对象,根据随机数字表法将其分为对照组和观察组,两组患者均接受常规静脉输液治疗。对照组在此基础上给予复方异丙托溴胺雾化吸入,观察组在此基础上给予糖皮质激素联合复方异丙托溴胺雾化吸入。观察两组患者的治疗效果,比较两组患者治疗前后肺功能、炎症因子水平的差异。结果观察组治疗有效率为98.33%,明显高于对照组(=5.886,P=0.015);两组患者治疗前炎症因子水平无差别,治疗后,观察组IL-8、hs-CRP和TNF-α水平低于对照组(t=57.536、5.270、9.986,P<0.001);两组患者治疗前肺功能指标无差别,治疗后,观察组肺功能优于对照组;两组患者治疗前血气指标无差别,治疗后,观察组Pa O2和PH水平高于对照组,Pa CO2水平低于对照组。结论糖皮质激素联合复方异丙托溴胺对AECOPD患者有较好的治疗效果,可明显改善患者的肺功能和炎症因子水平,具有良好的应用价值。 |

| Effect of Glucocorticoid Combined with Ipratropium Bromide in the Treatment of AECOPD and Its Effect on Serum Inflammatory Cytokines |

| Objective To investigate the effect of glucocorticoid combined with ipratropium bromide on patients with acute exacerbation of chronic obstructive pulmonary disease( AECOPD). Methods AECOPD patients treated in our hospital from January 2016 to December 2016 were selected as the research objects.According to the random number table method,they were divided into control group and observation group.Both groups received routine intravenous infusion.The control group was given compound ipratropium bromide inhalation on this basis.The observation group was given glucocorticoid combined with ipratropium bromide inhalation on this basis.The therapeutic effects of the two groups were observed,and the differences in the levels of pulmonary function and inflammatory cytokines between the two groups before and after treatment were compared.Results The effective rate of the observation group was 98. 33%,which was significantly higher than that of the control group( = 5. 886,P = 0. 015). There was no difference in the levels of inflammatory cytokines between the two groups before treatment. After treatment,the levels of IL-8,hs-CRP and TNF-α in the observation group were lower than those in the control group( t = 57. 536、5. 270、9. 986,P <0. 001).There was no difference in the index of pulmonary function between the two groups before treatment. After treatment,the lung function of the observation group was better than that of the control group. Before treatment,there was no difference in blood gas index between the two groups.After treatment,the Pa O2 and PH levels in the observation group were higher than those in the control group,and the Pa CO2 level in the observation group was lower than that in the control group. Conclusion Glucocorticoid combined with ipratropium bromide has a good therapeutic effect on patients with AECOPD,which can significantly improve the level of pulmonary function and inflammatory cytokines in patients with AECOPD,which has a good value |
