| 目的探讨术前外周血中性粒细胞与淋巴细胞比值(NLR)、肿瘤间质炎症反应与胃癌患者根治术后总体生存时间(OS)的相关性。方法收集67例患者资料,按NLR是否大于3分为低NLR组及高NLR组。高倍镜下评估肿瘤间质中性粒细胞、单个核细胞浸润程度和浸润细胞的主要类型,分别按浸润程度及类型分组。分析NLR、肿瘤间质炎症细胞浸润特点与临床病理特征的关系,以Kaplan-Meier生存曲线分析各指标与患者预后关系。结果 NLR与肿瘤标准物CA199、肿瘤大小、淋巴结转移和TNM分期密切相关,肿瘤间质以中性粒细胞浸润为主与肿瘤浸润较深、伴淋巴结转移显著相关(P<0.05),肿瘤间质中性粒细胞浸润程度与肿瘤浸润深度、淋巴结转移、TNM分期显著相关,但中性粒细胞/淋巴细胞比值与局部炎症反应无显著联系(P>0.05)。高NLR组患者OS较低NLR组明显缩短,中性粒细胞浸润程度高的患者OS明显缩短(P<0.05)。结论 NLR和肿瘤间质中性粒细胞浸润程度对胃癌患者预后有预测价值。 |

| Objective To discuss the relevance between preoperative peripheral blood neutrophils to lymphocytes ratio(NLR) with tumor stroma inflammatory response and overall survival(OS) in patients with gastric cancer after radical surgery.Methods 67 patients were divided into two groups according to whether NLR is greater than 3 scores.One was low NLR group and the other was high NLR group. The infiltration degree and cell main type of tumor stroma neutrophils and mononuclear cells under high power lens were evaluated and which were divided according that.The relationships between NLR,tumor stroma inflammatory cell infiltration characteristics and clinical pathologic features were analyzed.The Kaplan Meier-survival curve was used to analyze the relationship between each indicator and prognosis of patients.Results NLR was closely related to tumor marker CA199,tumor size,lymph node metastasis and TNM stage.Tumor stroma was mainly infiltrated with neutrophils and which was significantly related to deeper tumor infiltrating and lymph node metastasis(P < 0. 05). The neutrophils infiltration degree in tumor stroma was significantly related to tumor invasive depth,lymph node metastasis and TNM stage,but the neutrophils to lymphocytes ratio was of no significant relation to local inflammatory response(P>0. 05).Patients in high NLR group had lower OS than low NLR group and patients with high degree of neutrophils infiltration had obviously shortened OS(P< 0. 05).Conclusion NLR and tumor stroma neutrophils infiltrating degree are of predictive value for the prognosis of patients with gastric cancer. |
