| 目的分析单倍体造血干细胞移植治疗合并重症感染的极重型再生障碍性贫血的安全性及有效性。方法回顾性分析我科接受单倍体造血干细胞移植的2例合并重症感染极重型再生障碍性贫血患者的临床资料。预处理方案:氟达拉滨(FLU)+环磷酰胺(CTX)+抗人T细胞兔免疫球蛋白(ATG)。移植物抗宿主病预防:短程甲氨蝶呤(MTX)、环孢素(CSA)、骁悉(MMF)联合方案。移植物:动员后的骨髓及外周血干细胞。结果 2例患者均获得造血重建,中性粒细胞植活时间分别为12d和9d,血小板植活时间分别为18d和12d。随访时间分别为:32个月和30个月。结论单倍体造血干细胞移植可以作为合并重症感染的极重型再生障碍性贫血患者的一种治疗选择,但目前病例数较少,还需临床进一步探索及研究。 |

| Objective To evaluate the safety and efficacy of haploidentical hematopoietic stem cell transplantation for extremely severe aplastic anemia complicated with severe infection. Methods The clinical data of 2 extremely severe aplastic anemia patients complicated with severe infection who underwent hematopoietic stem cell transplantation in our department were retrospectively analyzed. The prepared processing scheme: Fludarabine(FLU) + Cyclophosphamide(CTX) + Rabbit immune globulin(ATG) against human T cells.Preventionof graft-versus-host disease: short-term of combined regimen of Methotrexate(MTX),Cyclosporine(CSA),Mycophenolate Mofetil(MMF).Grafts: bone marrow and peripheral blood stem cells after mobilization. Results Hematopoietic reconstitution was achieved in 2 patients.The neutrophil engraftment time was 12 d and 9 d,and the platelet engraftment time was 18 d and 12 d,respectively. And the follow-up time was 32 months and 30 months,respectively. Conclusion Haploidentical hematopoietic stem cell transplantation can be used as a kind of treatment option for extremely severe aplastic anemia patients with severe infection. However,the number of cases is small now and it needs further clinical investigation and research |
