目的探讨腹水中钙卫蛋白与总蛋白比率对诊断肝硬化自发性腹膜炎的临床意义。方法收集2014年2月到2015年8月在我院住院的127例乙肝肝硬化患者腹水,分为实验组及对照组,检测腹水中钙卫蛋白、中性粒细胞、白细胞及白蛋白含量,进行相关性分析。另外计算腹水中钙卫蛋白及总蛋白之比,绘制ROC曲线。结果
127份标本中,腹水中钙卫蛋白与腹水中白细胞、中性粒细胞及总蛋白之间存在相关性,实验组腹水中钙卫蛋白水平为126ng/m L~10842ng/m L(中位数928ng/m
L),在对照组,腹水中钙卫蛋白水平为26ng/m L~602ng/m L(中位数58ng/m L),差异有统计学意义(P<0.05),腹水中钙卫蛋白与总蛋白比率在自自发性腹膜炎诊断中可能更优于钙卫蛋白。结论腹水中钙卫蛋白与总蛋白比率可能是诊断乙肝肝硬化自发性腹膜炎的一个良好指标。 |

Objective To investigate the clinical significance of the ratio of calprotectin to total protein in ascites in the diagnosis of liver cirrhosis complicated with spontaneous peritonitis.Methods The ascites of 127 patients with hepatitis B cirrhosis hospitalized in our hospital from February 2014 to August 2015 were collected and divided into experimental group and control group.The levels of calprotectin,neutrophil,white blood cell and albumin in ascites were measured and the correlation between them were analyzed.In addition,the ratio of calprotectin and total protein in ascites was calculated,and the ROC curve was drawn.Results In the 127 specimens,there was a correlation between the levels of calprotectin and leukocytes,neutrophils and total proteins in ascites. The level of calprotectinin ascites in the experimental group ranged from 126 ng/m L to 10842 ng/m L( median928 ng/m L),while in the control group,the level of calprotectin in ascites was from 26 ng/m L to 602 ng/m L( median 58 ng/m L),and the difference was statistically significant( P<0. 05).The ratio of calprotectin to total protein in ascites may be better than that of calprotectin in the diagnosis of spontaneous peritonitis.Conclusion The ratio ofcalprotectin to total protein in ascites may be a good indicator for the diagnosis of spontaneous peritonitis in patients with hepatitis B cirrhosis |
